OFFICE POLICIES

Prior to your initial appointment with us, you will need to complete paperwork online through one-time access to our patient portal. We will provide all the information you will need once you provide your contact information.

Payments

All payments/fees and copays are due at the time of your appointment prior to seeing the doctor or clinician. For your convenience, we accept Visa, MasterCard, American Express, Discover, and Personal checks

Insurance

We accept the following insurances

  • Bcbs
  • Medicare
  • Bcn
  • Hap
  • Priority health

WE DO NOT ACCEPT MEDICAID

Patients are responsible for being aware of current insurance coverage and providing new insurance changes before any appointments. This includes

  • Out of network benefits
  • Deductible and or “out of pocket costs”
  • Current coverage and copayment
  • Any changes in coverage
  • Prescription coverage

You are responsible for any uncovered cost for your treatment and your mental health coverage may be “carved out” to other managed care companies, we may be considered out of network with these companies.

A complete list of office policies will be provided when your new patient appointment is made.

Cancellation policies: Because the appointment time has been reserved for you, we do impose a fee for appointments missed or canceled with less than 24 hours’ notice. These charges are not covered by your insurance and must be paid prior to any further appointments. We do NOT make appointment reminder phone calls, but as a courtesy, you will receive an email or text reminder two days prior to your appointment if you provide an email address. However, it is ultimately your responsibility to mark the date on your own calendar.

Medication Refills: It is the policy of our practice to provide our clients with enough medication to last until the next appointment. We may elect to refill your medications early in certain circumstances and on a case-by-case basis. We require 72-hour notice for all medication refills.
